Tailbone Pain in Pregnancy: What Relieves Pelvic Pressure

If your tailbone aches during pregnancy, the usual culprits are sitting pressure and the ligaments around your pelvis loosening up. A wedge cushion can take weight off that sore spot, and gentle strength work helps support the pelvis over time. If the pain followed a fall, or comes with bleeding or other warning signs, call your provider first.

I’m Norma Wardell, RN, LMT, BCTMB, and I’ve been a massage therapist since December 2003. Before you buy a cushion or book a massage, I’d like you to notice exactly where it hurts. Pain sitting right over the tailbone needs a different approach than an ache that runs deeper into your hip, and sorting out which one you have makes everything else easier.

What Usually Causes Tailbone Pain in Pregnancy?

Think of your tailbone, or coccyx, as the small cluster of fused bones at the very bottom of your spine. Muscles and ligaments hook it to the rest of the pelvis, including the pelvic floor, the sling of muscles that holds up your pelvic organs. So when this area hurts, the bone is not always the only thing involved.

Pressure from sitting

If a hard seat brings on the ache and standing eases it, check your sitting position first. Leaning back onto the base of your spine can put pressure directly on the tender area.

Looser ligaments as the pelvis changes

Pregnancy hormones allow the ligaments around your pelvis to become more flexible. That extra movement can leave the joint at the base of your spine feeling sore.

Muscles that support the pelvis

Your glutes, the muscles in your buttocks, help steady the pelvis. Tension or weakness in the supporting muscles can contribute to discomfort, which is why easing tension and building strength both matter.

An earlier fall or bump

Tell your provider if the pain began after landing on your bottom, even if the injury happened some time ago. Ongoing tenderness after a fall deserves an assessment before massage therapy.

Why Can the Ache Build as Pregnancy Progresses?

Relaxin and other pregnancy hormones loosen ligaments to help the pelvis accommodate pregnancy and birth. This can allow more movement where the sacrum, the broad bone at the back of your pelvis, meets the tailbone. As your belly grows, the demands on the supporting muscles change too.

These changes continue through pregnancy, so relief may come in stages. You do not need to accept worsening pain as inevitable. Discuss it with your provider, and read our prenatal massage safety guide for massage considerations during pregnancy.

How Can You Tell Whether It Is the Tailbone or the Hip?

Try this: put a finger on the sore spot. Is it right over the bone, or does the ache sit deeper, down in your buttock? Pain over the tailbone that gets worse when you sit usually points to pressure on that region. Deeper pain, especially if it travels down your thigh, may involve muscles or nerves around the pelvis.

These clues help you describe the pain; they do not establish a diagnosis. Tell your doctor or midwife when it started and whether a fall preceded it. New numbness or weakness needs prompt medical attention.

Why Can a Donut Cushion Leave Your Tailbone Sore?

A hole in the middle of a cushion seems like an obvious solution, but a ring-shaped cushion can shift weight onto your hips and lower back without adequately unloading the tailbone. If yours leaves you just as tender, do not assume you need to keep using it.

A wedge-shaped cushion often works better because it tips your pelvis slightly forward. That helps you rest on your sit bones, the two bony points beneath your buttocks, rather than the base of your spine. Choose a position that reduces discomfort instead of forcing yourself into one that looks correct.

Give your car seat the same attention you give your chair at home. Before a long drive around Cape Cod, adjust the seat and plan a stop where you can walk for a few minutes. I’d rather you shift position early than wait until sitting has already become painful.

Do You Have to Lie on Your Side for Prenatal Massage?

No, you don’t. At Moontide Massage & Wellness, we use the body cushion system, a bolstering setup rather than a belly cutout hole. It supports you so you can lie face down when that suits you.

If side-lying bothers your hip, tell your practitioner. You do not have to remain in an uncomfortable position for the session. Our prenatal massage safety guide explains more about positioning and preparation.

What Can Gentle Prenatal Massage Do for the Area Around the Sacrum?

Massage therapy can ease tension in the muscles around the back of your pelvis. This includes the piriformis, a small muscle deep in the buttock that helps turn your thigh outward. Reducing muscle tension may make movement more comfortable, but massage does not correct every cause of tailbone pain.

I’ll be honest with you: there is no set number of sessions that will resolve a pregnancy ache. Your body keeps changing, so sustained care may be needed over months rather than a single visit. Yes, you will almost certainly feel noticeable relief and move more comfortably right after a massage. But if an underlying muscle imbalance or weakness is contributing to that pain in the first place, massage alone cannot permanently resolve it. Hands-on therapy calms tight, guarded tissues and restores mobility, but targeted strength training is what builds the active support needed to stabilize your joints and keep the ache from returning.

How Sessions Build Over Time

At Moontide Massage & Wellness, we keep treatment notes for every client. Our therapists use those notes across sessions to learn your needs and preferences, including what helped and what felt uncomfortable. Tell your practitioner how sitting felt afterward, not just how you felt on the massage table.

What to Tell Your Practitioner First

Share your trimester and health history, including any bleeding, dizziness or blood pressure concerns. Your practitioner also needs to know whether your doctor or midwife has cleared you for massage.

Our team is licensed and insured, and clients are encouraged to try different therapists within the practice to find the right fit. You can also select a quiet session or indicate that conversation is welcome when booking. Our prenatal massage page explains the service.

What Daily Habits and Strength Work Keep the Pressure Off?

Start with the pressure. Cut down on long stretches of sitting, then ask your provider which strengthening exercises suit your pregnancy. What we’re after is a pelvis that feels supported, not one you have to push through pain to use.

  • Use a wedge cushion if it helps you sit comfortably on your sit bones.
  • Break up long periods of sitting with standing or walking.
  • Include approved strength work for your glutes and the muscles that turn your hips and support your trunk.
  • Check with your provider before beginning a new exercise program.

If an exercise increases the ache, stop and ask for guidance. A pelvic health physical therapist can assess how your muscles are working and help you choose exercises rather than guessing which area needs strengthening.

When Should You Call Your Provider Right Away?

Some symptoms are not a cushion or massage problem at all. If you have a fever, any bleeding, or fluid leaking, call your doctor or midwife promptly. The same goes for pain that keeps getting worse, numbness or weakness in your legs, pain or burning when you urinate, or a fall that left your tailbone sore. It can feel like a lot to call about, but these are exactly the calls your provider wants to hear.

Please hold off on massage therapy until your provider has looked into these concerns and told you whether massage is safe for you right now.

Postpone massage therapy until your provider has assessed these concerns and advised you about receiving massage.

How Do You Start If You Want Hands-On Help?

Start with your doctor or midwife. Jot down when the pain shows up and what changes it, like standing up or switching cushions. Bring those notes to your appointment so the conversation starts with what you are really feeling.

Moontide Massage & Wellness is at 1550 Falmouth Rd., Suite 4C, in Centerville. Call 508-776-1305 or use our contact page if you have questions before choosing an appointment. Once your provider has cleared you, you can book a prenatal massage online.

Frequently asked questions

What could be causing my swollen tailbone?

Visible swelling near your tailbone needs a medical assessment, especially after a fall or if the area is red or warm. Sitting pressure can cause tenderness, but it does not explain every lump or swelling. Contact your provider rather than assuming this is a routine pregnancy ache.

Is tailbone pain normal at 4 weeks pregnant?

Tailbone pain at four weeks should not automatically be attributed to pregnancy. Sitting pressure or an earlier injury may contribute, but your provider can assess the cause. Call promptly if pain worsens or comes with bleeding, fever or fluid leaking.

Why do I feel pain right at the top of my tailbone when sitting?

Pain in that area may involve the tailbone or nearby soft tissue. Notice whether it feels directly over the bone and whether standing reduces it. Tell your provider about any swelling or injury, and seek prompt advice for worsening pain or fever.

How can I manage pain caused by coccydynia?

Coccydynia means tailbone pain. A wedge cushion and regular breaks from sitting can reduce pressure, while provider-approved strengthening helps support the pelvis. Prenatal massage may ease surrounding muscle tension after medical clearance, but while massage feels good, massage therapy cannot replace strength training when muscular imbalances contribute to pain.
